The following application forms are now available for download. Completed forms cannot be submitted electronically at this time; applicants must submit the application forms and pay required fees in person at the Department offices.

Please be advised that no more than one, one year time extension will be granted to approved use permits that are not used within the period specified in the permit. Use permits include oak tree permits, conditional use permits and parking permits.
